The Traumatic Brain Injury (TBI) program is designed
to help Vermonters with a moderate to severe traumatic brain injury
find a community based living situation among family and friends. The
program provides rehabilitation and support to individuals to help
them maximize their independence and ultimately return to work.
Eligibility
The TBI program is available to Vermonters who:

Are 16 years of age or older.
- Are receiving traditional
Medicaid.
- Have a documented recent moderate to
severe brain injury.
- Require one-on-one instruction focusing
on independent living.
- Demonstrate a potential
for independent living and the possibility of
returning to vocational activities.
Services
The TBI program provides rehabilitation services, community supports
including 24 hour care and supervision, crisis support,
intensive counseling, medication review and instruction,
employment supports to assist
a person in finding and keeping a job, and other
individualized supports and services.
