VocRehab Vermont (VR) assists Vermonters with disabilities
who are receiving financial assistance from the Department
of Prevention, Assistance, Transition, and Health Access (PATH,
formally Social Welfare.)
Individuals on welfare who are identified as having a disability that is a barrier to becoming employed are referred by PATH to VR for services. These individuals are assigned to a specialized VR counselor that also serves as the person's PATH case manager. A single counselor then provides all services, for PATH and VR. As in the general VR program, the VR counselor assists the individual in identifying strengths, interests, skills, and barriers. A plan is developed to address the services needed to achieve the person's vocational goals.
Having a single case manager allows for flexibility and an individualized approach to assisting individuals with disabilities find meaningful work. This partnership with PATH allows individuals with disabilities participate at a level that is appropriate for them and supports a person through the process of preparing for and entering the workforce.
