The referral is the beginning of the VocRehab process. 

If you call (866-VRWORKS), visit our offices, or if this website is your first stop, VocRehab will ask you to fill out a short referral form with your name, address, and basic information.

Next, an appoinment with a VocRehab counselor will be scheduled.  In some VocRehab offices, a group orientation session is offered to people interested in VR services prior to seeing a counselor.

Once you have made the first step to call, stop in to an office, attend an orientation, or complete the referral form and email it to VocRehab, a counselor in your local office will meet and talk with you about your interests and abilities.

There are two options to complete the referral form ahead of time.  

You can either print the .pdf form, complete the form by hand and bring it with you to the VocRehab office,

or complete the form on the computer and attach it in an email to us ahead of time.

If you prefer, you can call or visit a VocRehab office to complete the Referral form.
