VocRehab Reach-Up Program

 

  VocRehab Vermont (VR) Reach-Up program  assists Vermonters with disabilities who are receiving financial assistance from the Department for Children and Families; Economic Services Division (ESD) formerly known as PATH.

Individuals receiving Reach Up, who are identified as having a disability that is a barrier to becoming employed, are referred by ESD to VR for services. These individuals are assigned to a specialized VR counselor that also serves as the person's ESD case manager. A single counselor then provides all services, for ESD and VR. As in the general VR program, the VR counselor assists the individual in identifying strengths, interests, skills, and barriers. A plan is developed to address the services needed to achieve the person's vocational goals.

Having a single case manager allows for flexibility and an individualized approach to assisting individuals with disabilities find meaningful work. This partnership with ESD allows individuals with disabilities to participate at a level that is appropriate for them and supports a person through the process of preparing for and entering the workforce .
